Fatty hepatosis: causes, symptoms, diagnostics

What is fatty hepatosis or steatohepatitis? According to their purpose, liver cells (hepatocytes) take an active part in metabolism, including fat metabolism. Unfortunately, the liver has the property of fat accumulation. At first, fat is simply collected in hepatocytes, but over time, liver cells get overfilled and burst, for which reason fat spreads throughout the liver tissues. This leads to the replacement of healthy cells with fibrous mass, functional liver decrease, deterioration of its performance, which is why the body is not cleaned of harmful substances. Over time, fibrosis develops into liver cirrhosis, and the organ becomes incompetent.

Causes of fatty hepatosis:

  • Excessive body weight (body mass index is over 30, the probability of hepatosis is about 40 %)
  • Diabetes mellitus (in up to 60 % of people with this disease, hepatosis developed)
  • Increased blood cholesterol level
  • Increased blood lipid level
  • Alcohol abuse
  • Use of aggressive, toxic medicines

Unfortunately, in most patients, fatty hepatosis is not diagnosed at the primary stage, since its symptoms do not disturb the patient’s comfort, so they do not apply to the doctor. For example, heaviness, discomfort in the liver, bitter taste in mouth, flatulence, nausea, etc. The diagnosis is made when serious changes have already begun to occur in the liver tissues.

For the fatty hepatosis diagnosis, the following examinations can be prescribed:

  • Biochemical blood test (ALT, AST)
  • Liver ultrasound allows to evaluate the increase in its size, as well as the heterogeneity of tissues affected by fatty hepatosis
  • Liver elastometry that allows to assess the degree of fibrous liver damage

Classification

Depending on its cause, fatty hepatosis is divided into:

  • Alcoholic liver disease (alcoholic fatty hepatosis), the cause is alcohol abuse
  • Non-alcoholic liver disease, the cause is obesity, diabetes, etc.
  • Drug-induced steatohepatitis due to negative effect of drugs.

There are also several stages of fatty hepatosis:

  • Fatty hepatosis of the 1 stage (33 % of hepatocytes in the field of vision are affected)
  • Fatty hepatosis of the 2 stage (fat drops of different sizes are contained in 33–66 % of hepatocytes)
  • Fatty steatohepatitis of the 3 stage (more than 66 % of hepatocytes are affected)
