What is liver fibrosis?

Damage to liver cells (hepatocytes) by hepatitis B, C, D, alcohol, etc. leads to the growth of connective tissue in the liver, where it no longer performs its functions. This process is called liver fibrosis. Without proper treatment, which would prevent the growth of connective tissue in the liver and restore damaged cells, fibrosis progresses and leads to cirrhosis.

Liver fibrosis: Symptoms and diagnostics

Often, liver fibrosis is asymptomatic, and it is impossible for the patient to self-identify it (especially at the initial stages). Sometimes, there is a decrease in performance efficiency, rapid fatigability, rapid emergence of bruises, haematomas. Explicit clinical manifestations can appear in 5–10 years — increased spleen, portal hypertension, stomach and oesophagus haemorrhage, decreased number of leukocytes. At the same time, the patient condition is relatively satisfactory.
To diagnose liver fibrosis and its stages, the following methods are mainly used:

  • Biochemical blood test (liver tests). The level of blood hepatic ALT and AST enzymes is evaluated
  • Liver ultrasound
  • Liver elastography (elastometry) to determine the severity of fibrosis
  • Liver biopsy (liver tissue collection to confirm the diagnosis; however, modern medicine use this method more and more rarely because of the presence of liver elastometry)

Liver fibrosis stages

There are 5 stages (grades) of liver fibrosis:

  • F0 — this stage is characterised by the absence of fibrosis, the liver is considered healthy.
  • liver fibrosis stage 1 (F1)
  • liver fibrosis stage 2 (F2)
  • liver fibrosis stage 3 (F3)
  • liver fibrosis stage 4 (F4) or cirrhosis

If the cause of liver fibrosis is viral hepatitis, the transition period from one stage to another (fibrosis proliferation) takes about 5 years on average. Fibrosis develops much faster at the last stages. In addition, the rate depends on whether the patient has got rid of a source of fibrosis (hepatitis virus or alcohol intake). The fibrosis degree is diagnosed by the liver elastometry.
